Q. A. List out all the potential hazards with regard to cylinder head lifting job during main engine overhaul.
B. As a second engineer, explain how do you carry out a risk assessment for the above-mentioned job.



This question may not have a specific answer, it depends on person to person, to make the answer general we shall try to find out points from the precautions we need to take while carrying out cylinder head overhauling.

Precautions we need to take before attending the job:
Ensure you have sufficient time
Obtain the immobilization certificate
Check and keep ready all conumable spares
Test and keep ready the required tools
Stopped engine
let it cool down sufficiently
Block the starting mechanism
Shut off starting air supply
Engage turning gear
Shut off cooling water
Shut off fuel oil
Shut off lubricating oil
Lock turbocharger rotors


A. List out all the potential hazards with regard to cylinder head lifting job during main engine overhaul.
Potential Hazards:
a. Inssufient time.
b. Hot surface.
c. Damage to the components.
d. Injury to personnel.
e. In appropriate lifting tools.
f. Unavailibility of spares.
g. Person involve in the job is inexperce.
h. Person involve in the job is tiered.
i. Surrouding is not clean and clear.
j. Person involved in multiple jobs.
k. Not wearing proper PPE.
l. splasing of lube oil and cooling water.
m. Turning of propeller while work in progress.

B. As a second engineer, explain how do you carry out a risk assessment for the above-mentioned job.
Prior starting a job a second engineer need to do a pre planing that invole a toolbox meeting. In the tool box meeting all the person involved in the job should paricipate and a breif description of the job should be given by second engineer. He should take feedback from all the members about their experience in the job and consiquently assign duties to them.
Now second engineer is able to identify the reisks involve in the job based on his own experience and feedback from the others, thus can carry out risk assessment.
Risk assessment involves the possiblities of happening something wrong and its effect on person, machinery, ship and environment.
Make a list of all the possible hazards which can cause an accedent. List all the precations necessary to minimize the possiblity of causing an accident. List all the preprations to minimize losses or damages if the accedent takes place. lets see with exmple.
a. In appropriate lifting tools. A lifting gear might seems to be in good condition and having sufficient strength. But in the absence of test certificate of the lifting gear you are not sure that you are using an appropriate tool. To minimize the chances of accident you sould check for the certificate and its velidity. In case the lifting gear failed while on load, make sure you minimize the damage by keeping all the persons in safe distance while the cylinder head is in lifted condition.
b. Person involve has no experience of the job. Make sure you have briefed him properly prior starting the job and he is confident to carry out the job, To avoid any accedent make sure you are supervizing him and in case an accident occurs the person would have given a job which involve the least damage.
Similary a second engineer can assess the risk for all the potential hazards.
